Can the disorder suddenly appear in a person who has had no symptoms before (excluding the irony)?

If they had symptoms before then the disorder was already there. Essentially, without the symptoms there is no disorder.

Symptoms of any disorder appear for the first time without prior symptom appearances. So the answer is Yes.

I think we can safely say that the first symptom to appear has not had any symptoms before it. :)
