What are the most important differences between the two?

Wegener's granulomatosis is associated with the presence of classical anti-neutrophil cytoplasmic antibodies (c-ANCA, found scattered throughout the cytoplasm), while Churg-Strauss is associated with the presence of perinuclear or protoplasmic ANCA (p-ANCA, found mainly near the nuclear membrane). The antigen in WG is typically proteinase 3, while in CS it is usually myeloperoxidase. They otherwise have extremely similar symptoms.
